Student Documentation

"I have been teaching emotionally disturbed children for about 20 years. I have found throughout the years that is very helpful to design a notebook on each child. In the notebooks each section is divided by dividers and labeled with the appropriate section. I have the sections broken into these categories: Behavior checklists, parent-teacher notes, documentation of physical restraint, medication chart, doctor releases, permission to release information to other agencies, psychologicals, progress reports, tardy and sick notes, work samples, permission slips for field trips, IEP. This is a very comprehensive way to keep your student's files and is very helpful when you go to IEP meetings. Organization is key in operating a smooth and efficient classroom."
